December 28,  2023 -  Viviant Health Care Shuttered by State

Viviant Health Care of Shelbyville,  a senior care facility,  was closed permanently by The Centers for Medicare and Medicaid Services on Tuesday.

The closure came after Viviant failed to comply with health and safety requirements for an extended period of time.

Earlier in December the Viviant facility in Murfreesboro was closed due to multiple violations and failure to pay over $300,000 in fines.

The Murfreesboro facility had been cited for failing to: provide and store food safely; develop comprehensive care plans for patients; have an infection prevention and control program; have ongoing assessments to review deficiencies and correct them; protect residents from wrongly using their belongings or money; keepings resident’s safe, clean and comfortable in a homelike environment; report Covid-19 data to residents and families; meeting residents needs in their first 48 hours; report abuse, neglect or theft in a timely manor; respond to alleged violations and safeguard resident information. 

The company also had similar violations in Bristol, TN and all indications are the company has been dissolved leaving hundreds of senior adult patients with no housing or medical care.  Local displaced residents will be relocated at othe available facilities over the next few weeks.  The Shelbyville Police Department is monitoring the location as a few apartment residents remain.
